Good Faith Estimate Notice

Effective Date: October 15, 2026

Last Updated: October 15, 2026

Your Right to Receive a Good Faith Estimate

If you do not have health insurance or do not plan to use health insurance to pay for your care, federal law generally gives you the right to receive a Good Faith Estimate of expected charges before receiving scheduled healthcare services.

A Good Faith Estimate explains what you are expected to be charged for the services included in the estimate. It is not a bill.

RehabDirect Physical Therapy, P.C. (“RehabDirect”) provides Good Faith Estimates to uninsured or self-pay individuals when required by applicable law.

1. Who May Receive a Good Faith Estimate?

You may have the right to receive a Good Faith Estimate if you:

  • do not have applicable health insurance or health coverage; or

  • have health coverage but do not intend to have a claim submitted to that coverage for the physical therapy services you are requesting.

If you are unsure whether these requirements apply to you, you may contact RehabDirect.

2. When Will You Receive a Good Faith Estimate?

You may request a Good Faith Estimate before scheduling physical therapy services.

Under current federal requirements:

  • if you request an estimate before scheduling services, it generally must be provided within 3 business days after your request;

  • if you schedule services at least 3 business days but fewer than 10 business days in advance, the estimate generally must be provided within 1 business day after scheduling; and

  • if you schedule services at least 10 business days in advance, the estimate generally must be provided within 3 business days after scheduling.

When care is scheduled fewer than 3 business days in advance, federal rules generally do not require a Good Faith Estimate before the service. You may still ask RehabDirect about the expected cost of your care.

Your Good Faith Estimate will be provided in writing, either electronically or on paper as appropriate.

3. What Will Your Estimate Include?

RehabDirect uses established self-pay rates for the physical therapy visit types offered by the practice. Current standard pricing is displayed on the RehabDirect website.

Your individualized Good Faith Estimate will identify the services included in the estimate and their expected charges. Depending on the care being scheduled, this may include:

  • the type of physical therapy visit;

  • the number of visits included;

  • the expected charge for each visit; and

  • the total expected charges.

If only an initial evaluation is scheduled, the estimate may cover only that evaluation. If additional services are later scheduled or requested, a new Good Faith Estimate will be provided when required.

Multiple or Recurring Physical Therapy Visits

When multiple physical therapy visits are expected, RehabDirect may provide one Good Faith Estimate covering the recurring visits rather than a separate estimate for every appointment.

When applicable, the estimate will describe the expected scope of care, including the anticipated timeframe, frequency, and total number of visits included.

Federal guidance permits a single Good Faith Estimate for recurring physical therapy services for a period of up to 12 months.

4. Additional Visits and Changes in Care

A clinical recommendation for additional physical therapy does not, by itself, require you to schedule or pay for additional visits.

If you choose to schedule visits that were not included in an earlier Good Faith Estimate, those visits will result in additional charges at the applicable rate. RehabDirect will provide a new or updated Good Faith Estimate when required based on the additional services being scheduled or requested.

The estimate reflects the services reasonably expected when it is prepared. Services that were not reasonably anticipated or that are later separately requested or scheduled may not be included in an earlier estimate.

5. A Good Faith Estimate Is Not a Bill or Commitment to Treatment

A Good Faith Estimate is intended to help you understand the expected cost of the services included in the estimate.

It is not a bill and does not require you to receive the services listed.

You remain free to decide whether to schedule or continue physical therapy. You are not financially responsible for additional visits merely because they are recommended if you do not schedule or receive them, except for any separate obligations that may apply under an agreed cancellation or other patient financial policy.

6. If Your Bill Is Higher Than Your Good Faith Estimate

Keep a copy of your Good Faith Estimate and compare it with any bill you receive.

If a provider bills you at least $400 more than that provider’s Good Faith Estimate, you may be eligible to use the federal patient-provider dispute-resolution process.

Under current federal requirements, the dispute generally must be started within 120 calendar days of the date on the initial bill.

Using the federal dispute-resolution process does not prevent you from contacting RehabDirect first if you believe there is an error or have questions about a charge.

7. Requesting a Good Faith Estimate

You may request a Good Faith Estimate before deciding whether to schedule care. Federal rules require providers to furnish an estimate when an eligible uninsured or self-pay individual requests one, even if services have not yet been scheduled.
